I am going through this link,but after implementing the form as given in this link my values at the right side of the form is Zero,it means every value is Zero at all the fields which are at right hand side…and also in date filter and item filter field no value is coming?

please suggest me.

You need to enter values in Date Filter and Item Filter manually…

Check the standard Form 554 for the same you will get an idea…
