How to get FORMNOTE on purchase order confirmation report?

Hi I have one requirement one of the my client

I want print formnote on purchase confirmation order report in ax 2012…

Suppose I have 100 vendors in that 95 vendors should print standard formnote … remaining 3 or 5 vendors should print standard formnote + extra text (this should be user editable ) and this form note should need add Purchtablelistpage form in action pane …

Please suggest me how to achieve this requirement …?

Do you want to add it to purchase order header, so that you can edit it? If so, List pages are meant for view not for editing.
You should be rather adding it PurchTable form (in header).
