Relavant record is not displaying of main account form for assigned work item of workflow

Hi All,

I have one custom workflow on main account form when i am submitting workflow it shows notification on home page for assigned work item and when i am clicked on work item it redirect to WorkflowWorkList form where it show all the work item. whenever i click on my workitem it shows blank main account instead of actual for which workflow submitted. Kindly check attached image.
