I’m sorry, but it’s impossible answer it unless you give us more information. First of all, please tell us to which form you’re trying to add the field.

We are adding these fields on Sales header form.
I’m don’t know any Sales header form. Do you mean that you go to All sales orders, open a sales order (= SalesTable form), switch to Header section and try to add a field there?
One address (the first one, I assume) is the delivery address. The other is Third-party shipping address, which is deprecated and you shouldn’t be using it.
If you’re interested in details, this address is at the Address tab too, but it’s never visible. To see it, you’d have to enable Shipping carrier configuration key, which isn’t allowed anymore. See Enabling Shipping carrier configuration key if you want to know why.
