Is there any standard way to have a Revision # and Revision Dates by SKU and also be able to print that information on a PO confirmation?
If there is also a work around , it will be good.

You can use the configuration dimension to indicate a version, but it depends upon the actual need. Printing the version on a PO could be as simple as having the version in the item description.
Thanks Adam but what are the pros and cos to use item configuration? I dont want to use something that can be overkill for a user. I have never used configuration before.
The overkill depends upon the importance of the revision and whether it becomes a distinct item, I would never use it, but to most people the revision is not important, it is just a reference. As a simple example the current BOM could hold the revision, or it could be a document note. Depends upon what the customer wants it to do and control, so you need to speak to them and understand the requirement and where it impacts on processing, stock on hand, planning etc.
From my understanding, the Customer want to make sure that item has always a reference when the manufacturer issue a new SKU.
A reference could be many things, show the options to your customer with the pro’s and con’s of each approach.
