We have had users create and post BOM RAF journals against BOMs that are not active. Is this a bug and is there a way to stop this without modification?

You need to unapprove it. No issues in many businesses of having multiple BOM versions, there is only one active BOM but any other approved BOM can be used - it is a user decision, they have to pick it to pick the correct configuration of BOM lines, but it is a choice. As default the active one is suggested.
